Risk Assessment


-each number corresponds to the same risk, danger, procedure and prevention

Risk
1.     Use of knife
2.     Allergic Reaction/Fake blood 
3.     Falling over equipment
4.     Electrical Fire
5.     Falling down staircase
6.     Slipping on plastic sheets
7.     Equipment could be stolen
8.     Slipping on fake blood mixture
Danger
1.     Could harm actors
2.     Actors may be allergic to ingredients in fake blood
3.     Actors/Crew could trip on cables for the camera/lighting
4.     Cables/Equipment/Chargers could cause fires
5.     Actors or cameraman could fall down staircase
6.     Actors could slip on plastic sheeting while setting up
7.     Equipment could be taken/stolen causing property loss
8.     Could slip on mixture and injure yourself
Procedure
1.     Call an ambulance and prevent bleeding
2.     Call and ambulance and take medicine 
3.     Make sure the person is ok/call ambulance
4.     Call the fire brigade and get away from the set
5.     Check for injury and call ambulance if injured
6.     Check for injury and call ambulance if injured
7.     Call police if occurs
8.     Check for injury and call ambulance if injured
Prevention
1.     Use a fake plastic/rubber duplicate 
2.     Make sure actor has no allergies
3.     Put tape over cables and tell actors where/not to walk
4.     Make sure all the electrics are in perfect condition, make sure nothing is being overloaded
5.     Ensure all watch and walk carefully down stairs
6.     Wear very good grip shoes and step carefully
7.     Always have some with equipment to ensure it is always watched
8.     Step carefully and don’t spill mixture
